THROMBOELASTOMETER-AUTOMATED (TEM-A)

A RELIABLE Rapid Method

A point of care testing for hemostasis, from coagulation to fibrinolysis.

TEM-A monitors hemostasis as a whole dynamic process which measures the viscoelastic and mechanical properties of a developing clot.

Referred as "Qualitative measurement" compared to conventional coagulation test which only produces "Quantitative measurement".

Rapidly detects any coagulopathy, and the risk of it's occurance while planning on transfusion therapy or blood product consumption.

The Technology

Base on the original Hartert's model & principle of thromboelastograph :

360μl sample of whole blood is placed into a pre-warmed (37oC) cup which oscillates through angle of 4o45' rotating every 10 seconds .

A plastic pin is suspended in the blood sample by a torsion wire and monitored for motions produced.

Electromagnetic transducer records various stages of in-vitro hemostasis including :

clot formation characterized by fibrin formation and aggregation

conventional strength of formed clot representing kinetics of fibrin build up

platelet function assessment

hyperfibrinolysis

TEM-A Tracings & Parameters Evaluation

Parameter

Description

R Time (R)

Period of time from initiation of the test to the initial fibrin formation

K Value (k)

Time from the beginning of clot formation until the amplitude of 20mm ( clot dynamic )

Alpha Angle ( a O )

Measures the speed of aggregation ( rapidity of fibrin build up & cross linking )

Maximum Amplitude (MA)

Represent the strenght of the clot which dependent on the number of platelets and its interaction with fibrin

"Thromboelastograph is a better predictor method with 87% accuracy in post-OP haemorrhage and re-exploration needs."

(Spiess & colleagues)

"Thromboelastograph-guided therapy would decrease the use of empirical therapy & excessive transfusion."

(Kang & colleagues)
